The teacher and support team respond to the student’s needs by following the process of Adjust:
Carry Out Diagnostic Testing
The teacher ensures the student has access to diagnostic testing to determine if they have a learning disability, what that disability is and the appropriate supports.

- The Head of Wellbeing coordinates for Specialists to conduct diagnostic tests on the student.
- The Head of Wellbeing informs the parents about the student’s impending diagnostic testing using the Visiting Specialist Assessment form
- The Head of Wellbeing supervises Specialists when they are in the school and ensures they have all relevant student information.
- The Specialist conducts diagnostic testing on the student.
- The Specialist provides Diagnostic Test Results to the Head of Wellbeing.
- The Head of Wellbeing provides a copy of the Diagnostic Test Results to the parents.
Review Assessment Results
The teacher ensures that all parties understand the nature of the student’s disability so they can collaborate on how the students can best be supported.
- The Teacher explains the Diagnostic Test Results to the group and answers any questions.
- The Student Support Team endorses that the student:
- has a disability that impacts their functioning in the school setting
- is eligible for special education services and requires an Individualised Plan or
- is not eligible for special education services and will return to receiving instruction in the general education setting.
Draft Individualised Plan
The teacher is always responsive to the emerging needs of students and takes action using data to draft an Individualised Plan to support the student’s learning across the school.
- The Teacher opens Individualised Plan template.
- The Teacher completes sections of student profile to Extent of Nonparticipation.
- The Teacher uses the Learning Goals Matrix to select one relevant goal for each area of intervention.
- The Teacher reviews the Adjustments Matrix and identifies and adds adjustments that would most benefit the student.
- The Teacher reviews the Modifications Matrix and identifies and adds appropriate modifications for the student, if applicable, to the Support Aides: Adjustments table.
- The Teacher completes the Testing Supports table.
- The Teacher completes the Transition Plan (if applicable).
- The Teacher shares the proposed Individualised Plan with the Head of Wellbeing for an expert opinion.
- The Teacher shares the proposed Individualised Plan with the Student Support Team in the meeting and:
- completes Parent Input section with parents during the meeting
- discusses and agrees on the Communication Plan with the Student Support Team
- gets all attendees to sign the Individualised Plan.
- The Teacher distributes Individualised Plan to any other teaching staff and Specialists who teach that student.
- The Teacher implements the Individualised Plan the next school day after endorsement.
