Review
The teacher and support team respond to the student’s needs by following the process of Review.
Review Student’s Data and Update Student Profile
The teacher collects testing and assessments on all students to understand how effective instruction has been and what adjustments are required.

- The Teacher uses the Review Checklist to collect data.
- The Teacher assesses student progress focussing on students’ completion of learning goals.
- The Teacher meets with the Head of Wellbeing to review the Student Data Report.
- The Teacher and Head of Wellbeing evaluate the implementation of the Individualised Plan using the Individualised Plan Evaluation and complete summary, learnings and recommendations.
- The Teacher and Head of Wellbeing share the Individualised Plan Review Report with the Student Support Team to understand the impact of the Individualised Plan.
- The Teacher and Head of Wellbeing determine if the student is demonstrating learning at the appropriate rate or if they suspect a learning disability.
- The Teacher calls a Student Support Team meeting to discuss intervention results.
- The Teacher shares the Student Data Report.
- The Teacher explains any detail parents are unclear on.
- The Student Support Team decides whether:
- the student has made the required growth so they can discontinue the intervention
- the student is having difficulty and will be assessed to determine if they have a learning disability.
Update Individualised Plan
The Individualised Plan is repeatedly reviewed and updated to reflect a student’s changing needs.
- The Teacher updates the Individualised Plan goals and adjusts the adjustments and modifications relevant to transitions during the implementation
- The Student Support Team endorses the updated Individualised Plan.
- The Teacher implements the updated Individualised Plan the following school day.
- The Teacher records notes and updates the Student Profile.
