The Process of Creating an Individualised Plan
An Individualised Plan is developed for students with a disability that impacts their functioning in the school setting. It addresses the academic, communication, or functional goals as well as the support and adjustments students require to meet their learning needs.
Student disabilities are unique to the student, but those with similar disabilities may have a similar Individualised Plan. There are differences between students and the adjustments set for them. As a student develops, their need for a particular adjustment may also change, so ongoing assessment is critical to ensure that adjustments align with student needs.

The Matrices
The Learning Goals Matrix is used by teachers to set the most appropriate learning goals for a student in their Individualised Plan. Goals focus on core skills that students need to master to succeed in the general curriculum.
The Adjustments Matrix is used to set the most appropriate adjustments for a student when completing their Individualised Plan.
The Instructional Strategies Matrix is used to identify instructional strategies that can be applied to teach or reteach specific, foundational skills. Teachers use the matrix to select the appropriate instructional strategies to teach students during intervention or as part of their Individualised Plan.
The Modifications Matrix is used to set the most appropriate modifications for students who require them, as they require changes to what they are expected to learn or produce while in the general classroom.
